How To Get Your Medical Marijuana Card
Step 1) Do You Qualify?

Make an appointment with a Doctor near you : Approved Physician List
Discuss your medical history with your MD or DO and identify which qualifying medical conditions will allow you to get your meical marijuana card.

- Epilepsy
- Glaucoma
- Crohn’s disease
Step 2) Create A Profile
Create Your Profile with the Department of Health
Register with DHSS Online
Visit the Department of Health: health.gov to create a profile. You’ll need to be able to prove residency in the state of Missouri.
Step 3) Apply For Your Card
Submit Application to Department of Health
Finalizing Application
Log-in to your newly created profile: health.gov
Upload your documentation
You will need to upload the signed Physician Certification Form from your MD or DO. Make sure you do this within 30 days of your doctor’s authorization.
Pay License Fee ($50.00)
You’ll have your medical marijuana card within 30 days of submission.
